Sleeping Tablets

What are sleeping tablets?

A medication that is taken to induce sleep is known as a sleeping tablet. They are also known as soporific and hypnotic drugs. These medications are used for treating insomniac patients (who have trouble sleeping and staying asleep).  These drugs have a sedative effect that calms the brain and relieves the anxiety. They can be used for both increasing onsets of sleep and sleep onset latency. These medications are a second line treatment for insomniac patients if cognitive therapy did not work. These medications help you to fall asleep rapidly and make you stay asleep Barbiturates, quinazolinones and benzodiazepines mostly fall under the category of sleeping pills. Other medicines include:

  • Antipsychotics (quetiapine)
  • Antidepressants (mirtazapine)
  • Melanin
  • Antihistamines

Out of all of the above medications, benzodiazepines are considered a safe drug treatment for the short term treatment of insomnia. Non-benzodiazepines are developed most recently and they are considered more effective and less toxic than benzodiazepines.

What causes insomnia?

Insomnia is a condition in which a person faces difficulty falling asleep, staying asleep, makes you wake up early and not able to get back to sleep.  Common causes of insomnia include:

  • Irregular sleep schedule
  • Poor sleep habits
  • Travel
  • Stress
  • Mental health disorders (depression, anxiety)
  • Medical conditions
  • Sleep disorders
  • Pain
  • Medications
  • Neurological problems
  • Eating late in the evening

Stress:

Work stress, exam stress, school stress, and financial stress can lead to insomnia. Or any other stress from trauma which could be both physical or emotional ( death of a loved one, separation) can make you stay up at night.

Poor sleep habits:

Poor sleep habits like waking up at night, staying asleep during the day, taking short naps during the day, watching TV, or using a mobile phone till late at night, and uncomfortable sleep environment, can also lead to the development of insomnia.

Travel:

Travel and different work schedules ( working late, morning shifts. jet lag) can disturb circadian rhythms and they can lead to insomnia.

Medications:

Certain medications can also interfere with the sleep-wake cycle. These medications include:

  • Painkillers
  • Asthma medications
  • Antidepressants
  • Anti-hypertensives
  • Anti-allergic medications

Sleep related disorders:

Some sleep related disorders also can cause insomnia. Sleep apnea (a condition that causes breathing repeatedly stop and start while sleeping). Restless leg syndrome can also cause tingling sensations in your legs during the night which can disturb your sleep.

Medical conditions:

Medical conditions can also disturb the sleep pattern. These conditions include:

  • Parkinson's disease
  • Alzheimer's disease
  • Cardiovascular problems
  • Chronic pain
  • Asthma
  • Gastroesophageal reflux disease (GERD)
  • Overactive thyroid
  • Cancer
  • Diabetes

Mental health disorders:

Mental health disorders like anxiety, depression, panic attacks, and post traumatic stress disorder can also lead to insomnia.

Eating late in the evening:

Intake of junk food, caffeine, nicotine, cold drinks, and alcohol in the evening or late at night can also lead to sleeping problems. These are the stimulants that disturb the sleep cycle. Alcohol may help in falling asleep, but it prevents deeper stages of sleep and can cause awakening in the middle of the night.

Insomnia becomes more common with increasing age. In older people it can occur due to medications, sleeping patterns, health changes, and changes in activities(less active both socially and physically) Moreover, it is more common in females in their menstrual cycle due to the changing of hormones.

How do sleeping tablets work?

Benzodiazepines work by increasing Gamma Aminobutyric acid (GABA) receptors in the CNS by binding to GABA receptors which slows down the CNS ultimately causing a sedative and relaxing effect on the brain. They cause Central Nervous System depression.  Barbiturates also work in the same manner by affecting inhibitory chemicals in the brain. They absorb rapidly in your bloodstream and start working within one hour and reaching peak concentration in 1-1.5 hours. They crossed the blood brain barrier rapidly. So, the patient will start to feel calming and relaxing effects within an hour or two. Their effects may last for more than 24 hours.

Types of Insomnia treatments are available

There are so many treatment options available for treating insomnia which include both pharmacological and non-pharmacological management.  Pharmacological management includes:

  • Melatonin agonist (Luzindole)
  • Non-benzodiazepines
  • Zolpidem
  • Orexin receptor antagonist
  • Eszopiclone (Lunesta)
  • Ramelteon (Rozerem)
  • Zaleplon (Sonata)
  • Antihistamines

Certain anti-depressants and anti-psychotic medications are also helpful in treating insomnia. Melatonin agonists help in balancing the amount of melatonin in the body which will eventually have a calming effect on the mind and help in improving sleep quality and time

Non-pharmacological management includes:

  • Cognitive behavior therapy
  • Light therapy
  • Relaxation techniques
  • Regular exercise
  • Yoga
  • Breathing exercises
  • Stimulus control
  • Patient Education
  • Paradoxical intention
  • Stimulus control therapy
  • Sleep restriction and compression

Cognitive behavior therapy is considered as the first line treatment of insomnia because it is free of side effects. It helps you get rid of negative thoughts and tensions that keep you up at night. The behavioral part helps in developing good sleep habits. It includes the education of patients, which is the most important factor. Patients should be aware of their sleeping patterns. It focuses on promoting behaviors that improve the quality of sleep. Paradoxical intention can also help in inducing sleep. It encourages the patient to engage in his most feared behavior. Relaxation techniques can also help to achieve the goal of good sleep quality by reducing anxiety and stress.  It includes breathing exercises, performing yoga, and biofeedback (therapy that helps you control body processes such as blood pressure and muscle tension). As we know exercise plays a major role in relaxing brain chemicals. Aerobic exercises are considered to have the same effect as sleeping pills. Furthermore, light therapies can help in inducing sleep. Dim and blue lights help in the rapid onset of sleep.

Some preventions can also lead to an improvement in condition. They include:

    • Avoid taking naps during the day.
    • Be active (Regular activity helps encourage a good night's sleep).
    • Avoid or limit caffeine and alcohol.
    • Avoid nicotine.
    • Don't eat heavy meals before bedtime.
    • Don't drink cold drinks before bedtime.
    • Check your medications to see if they can cause  insomnia.
    • Keep your bedtime and wake time steady from day to day, including weekends.
